Humana launches behavioral health as component of CKD and ESRD Disease Management programs

Like most conditions, patient behavior plays a significant part in the health of a member with CKD and ESRD. By including behavioral health support, we are able to better identify and manage these members to improve outcomes.

Beginning June 1st, 2008, Humana is improving our current Chronic Kidney Disease (CKD) and End Stage Renal Disease (ESRD) Disease Management programs by enhancing a behavioral component.

How will eligible members be identified?
The health screening that is part of the initial assessment for eligibility in CKD or ESRD programs has been enhanced to assess a variety of behavioral health symptoms. Based on the information gathered during the assessment, behavioral health case management may also be provided. Additionally, enrolled members who are already in the CKD or ESRD programs will be re-screened based on their annual reassessment date. All eligible members will be re-screened and offered the behavioral health component by the end of the year.

The enhancement is being offered to eligible members at no charge to Commercial, Medicare, and ASO customers as part of their existing CKD and ESRD Disease Management programs.
